Extra fields not showing on registration form

HI,
The extra fields I defined on AJAXRegister are not showing up on the registration form. The fields in question are: Province - dropdown, Gender - dropdown, Subscription - multiple radio buttons.
What needs to be done?

If you followed the documentation it should show. It can be many things, best to send an email with admin access so I can have a look.
