I see quite a few requests for mydomain.com/favicon.ico ... please advise how to fix this?
edit - this is solved I guess.. stupid thread by me lol ??? :

Some browsers send request for favicon.ico on the root regardless of its directory setup in header. So it's best to have a copy on root as well.

As for other URLs that generate 404 (when they shouldn't), can you give us a brief outline of things/steps to check for?
For example, these two URL's don't work:
/news/mobile-news/mouse-atc-_-advanced-translational-control.html
/news/mobile-news/it's-official:-dopod-providing-wm6-upgrades.html
I have -=>_ as replacement, and I have added ' as an exception.

I can't without looking at your exact settings in SEF Advance. It can also behave differently on different servers and depend on server settings like magic quotes etc.
It's best not to except any chars that make problems and let them get URL encoded.
