In the Url Replace I have entered
:10500=>treatment
to replace the :10500 in urls such as
www.adhdtreatment.org:10500/adhd/reviews...from_amazon.com.html
But nothing happens.
What am I doing wrong?

You can't replace that with SEF Advance. Looks like a port number, it's not SEF Advance that generates it, don't know where it comes from.

Inorder to have my own Apache (which is faster ) on the shared server, I need that port number. I knew that SEF Advance was not generting it I was just hoping the SEF Advance could change it into something more useful;

No, SEF Advance can change the URL paths only, not the actual domain/port which lies on server level.
You might be able to do something with .htaccess but not sure about that.

I am having a similar problem replacing %26%238230; in
www.adhdtreatment.org:10500/adhd/the_tyr...es_sure_he_wont.html
I am using %26%238230;=> since I just want it to leave it out.
It is translating ellipses ... in the page title into %26%238230;

You can't leave out a part of URL. You can replace it with something else but it is very sensitive as you must think the other way around. It is very well explained in documentation and FAQ and I suggest you check that. But as a rule of thumb use it only for a single character replacement.
In your case you are better off using the alias. Set "Use title alias" to "Yes" and make sure your article aliases do not contain strange characters.
